- Complete the Membership Application (Download here);
- Include copies of your of your attestations, diplomas, credentials or certificates (and proof of hours when necessary);
- Sign and date the Agreement (page 1) and the Declaration (page 3).
Make payment of your membership according to one of the following choices:
- cheque or money order made out to the A.C.N.N; or;
- E-Transfer (please contact us by email at email@example.com in order to get the instructions).
Mail all documents with your payment to the address shown on the Form header.
You can also send a SCANNED copy of the Application Form with all documents by email at firstname.lastname@example.org.
