Procedure

• Complete the Membership Application (Download here). If you were a member of the ACNN in the past and you wish to reinstate your membership, please contact us at info@acnn.ca in order to obtain the Reinstatement Form and details;

• Include copies of your of certificates or diplomas (and proof of hours when necessary);
• If your certificates or diplomas are delivered in languages other than French, English, Spanish or Italian, they must be translated by an official translator;

• Sign and date the Agreement (page 1) and the Declaration (page 3);
• All documents can be sent by email to info@acnn.ca or by mail to the address indicated on the header of the Form (see payment instructions hereunder).

Make payment of your membership according to one of the following choices:

• By email: E-Transfer (We will send you payment instructions once we receive the completed form and documents).
• By mail: cheque or money order made out to the A.C.N.N;
